The new balance on the OTC Plus card can be used on items that promote a healthy lifestyle, from over-the-counter medicines to healthy foods, Internet services, and fitness equipment and trackers.

The OTC Plus card is available with the Healthfirst Life Improvement Plan and CompleteCare Plan.

Staying healthy is easier with a Healthfirst OTC Plus and OTC card. Save on items you use every day, like toothpaste, eye drops, aspirin and more, when you shop at participating local stores and select online retailers with free home delivery options.

With access to thousands of doctors and specialists, you're sure to find the service you need near you. All of usmunicipal officesis fully staffed with representatives who can answer questions, in many different languages, about our health insurance plans and programs, whether you're a member or not.

Healthfirst plans with an OTC Plus card:

1. Life Enhancement Plan (HMO D-SNP)
   $475/quarter allowance
2. CompleteCare-Plan (HMO D-SNP)
   $180/month allowance
3. High Performance Plan (HMO)
   $100/quarterly allowance

Unused credit automatically expires at the end of each month/quarter or when you disenroll from the plan.

Activate your card

You must provide the following:

• The OTC Plus/OTC card number on the front of the card
• Your Healthfirst Medicare Plan member ID card number
• Your date of birth (2-digit month, 2-digit day, and 4-digit year; for example, 05/20/1955)

Visit a participating location (in person or online)

Go to a participating store with your activated Healthfirst OTC Plus/OTC card and select the approved items you wish to purchase.

You can use your Healthfirst OTC Plus Card at the following locations:

• Minoristas como Duane Reade, Walgreens, CVS, Rite Aid, Family Dollar
• Many local independent pharmacies
• Home delivery options through NationsOTC or Mom's Meals
• GrowNYC Locations

Take your items to the boxes of local retailers. At checkout, swipe your OTC Plus/OTC card to make payment. Purchases of approved items are automatically deducted from your card balance.

Please bring your OTC Plus/OTC card as you cannot use your Healthfirst membership card or Medicare card to purchase OTC items.

When shopping online, remember to enter your OTC Plus/OTC Card16 digit number.
