The new OTC Plus card balance can be used on items that promote a healthy lifestyle, from over-the-counter medicines and healthy foods to internet services and fitness equipment and monitors.

The OTC Plus card is available with the Healthfirst Life Improvement Plan and the CompleteCare Plan.

Health insurance can help you stay healthy. Even if you are in good health, health insurance can give you access to free primary care, emergency treatment, and preventative services. You never know when an accident will happen or if you will get sick and have to go to the hospital. When that day comes, the costs may be prohibitive and you and your family may not be able to afford the care you need. That's where health insurance comes in. It's good for your health, your wallet and your future.

Healthfirst is a nonprofit community organization sponsored by some of New York's most prestigious and nationally recognized hospitals and medical centers. With over one million members and counting, Healthfirst has been serving the communities of New York City for more than 25 years.

With access to thousands of doctors and specialists, you're sure to find the service you need in your area. all of usmunicipal officesis fully staffed with representatives who can answer questions about our health insurance plans and programs in many different languages, whether you are a member or not.

Healthfirst plans with an OTC Plus card:

  1. Life Improvement Plan (HMO D-SNP)
    $475/quarter allowance
  2. Plan CompleteCare (HMO D-SNP)
    $180/month allowance
  3. Performance Improvement Plan (HMO)
    $100/quarterly allowance

Unused credit automatically expires at the end of each month/quarter or when you unsubscribe from the plan.

Step 1

Activate your card

You must provide the following:

  • The OTC Plus/OTC card number on the face of the card
  • Your Healthfirst Medicare membership card number
  • Your date of birth (2-digit month, 2-digit day and 4-digit year; for example 05/20/1955)

Visit a participating location (in person or online)

Go to a participating store with your activated Healthfirst OTC Plus/OTC card and select the approved items you wish to purchase.

You can use your Healthfirst OTC Plus Card at the following locations, among others:

  • Minoristas including Duane Reade, Walgreens, CVS, Rite Aid, Family Dollar
  • Many local independent pharmacies
  • Home delivery options via NationsOTC or Mom's Meals
  • GrowNYC locations
  • For information on using your OTC Plus card at farmers' markets, please visit the on-site information desk

pay for your items

Checkout your items at local retail stores. At checkout, swipe your OTC Plus/OTC card to complete payment. Purchases of approved items are automatically deducted from your card balance.

Take your OTC Plus/OTC card with you, as you cannot use your Healthfirst membership card or Medicare card to purchase OTC items.

When shopping online, remember to enter your OTC Plus/OTC card16-digit number.

What do I need to apply for Healthfirst insurance?

Here's what you need to apply for Healthfirst insurance:

  • Proof of age, e.g. B. Your birth certificate.
  • Proof of US citizenship or legal residency status
  • Proof of current income (e.g. payslip if you are employed)
  • Proof of income from one of the following sources:
    • Social Security
    • Supplemental Security Income (SSI)

When will I receive my membership card?

  • For all plans with a reward, your membership card will be mailed to you after the initial payment (also known as the binding payment) is made.
  • For all non-premium plans, your membership card will be emailed to you upon signup

What is New York State of Health (NYSOH)?

New York State has created a health insurance marketplace or exchange called the NY State of Health to help individuals and small businesses obtain health insurance. The NY State of Health website launched on October 1, 2013. Consumers can search for health plans such as Healthfirst Leaf Plans on the NY State of Health website.

Premium payments are due on the 1st of each month, with the exception of payments for children's health insurance, which are due on the last day of the month. You will receive an invoice with further details by mail every month.
